Do you need a COVID-19 vaccine card for travel? If you received your 1st COVID-19 vaccine shot with the Bear River Health Department, click HERE and fill out the COVID-19 vaccine card request form. The card will be available for pick up the next business day following the date of submission. If you received your 1st COVID-19 vaccine shot elsewhere please contact them for a replacement card.
Business hours for card pick up:
– Monday-Thursday, 12 pm – 6 pm (Logan Office & Brigham City Office)
– Wednesday & Thursday, 2 pm – 5 pm (Tremonton Office)
The new card will cost $3, payment is expected at the time of pick up.
